Billing Process
How billing works
- Your healthcare provider orders your test. If you plan to use insurance, your provider sends your insurance information with the order.
- Baylor Genetics reviews the order and billing information. What you may owe can depend on the test, insurance coverage, prior authorization requirements, and plan details such as your deductible and coinsurance.
- Our Billing team can explain what to expect during the billing process and discuss patient access options, including self-pay and financial assistance.
How much will this cost me?
The amount you may owe can vary. It depends on the test your provider orders, and your insurance benefits. An estimate is not a guarantee of coverage or your final cost.
If you plan to use insurance
We will automatically check your insurance benefits as part of the testing process. Most insurance estimates are available within 24 hours after your test order is placed. If you have questions about your estimate or coverage, our Billing team is available to help.
If you are not using insurance
Contact our Billing team at 800-411-4363 or email [email protected]. The team can share the self-pay price for your test and explain available financial options.
When can I expect my bill?
If Baylor Genetics bills your insurance and you are responsible for a cost, you will receive a bill after the insurance billing process is complete, including any applicable appeals. The amount of time it takes to receive a bill can vary based on the insurer but generally takes at least four to five months from the time the test completes.
If you use self-pay, contact our Billing team at 800-411-4363 or email [email protected] to understand the price and when payment is due.
Is an Explanation of Benefits a bill?
No. An Explanation of Benefits, or EOB, is a statement from your insurance company that explains how it processed a claim. It is not a bill. A Baylor Genetics bill will show the amount you owe.
If you have questions about an EOB or a Baylor Genetics bill, email [email protected] or call 800-411-4363 to reach our billing team.

Insurance & Coverage
Is Baylor Genetics in my insurance plan’s network?
Baylor Genetics participates with many commercial and government insurance plans. Network status can vary by plan, product, and location. Confirm Baylor Genetics’ network participation and coverage with your health plan before testing.
Regardless of network status, your insurer may apply a deductible, copayment, coinsurance, or another amount that you are responsible for paying.
What if my plan is not listed
The plan information shown here is updated as new information becomes available, but it may not reflect changes immediately. If your plan is not shown, that does not necessarily mean Baylor Genetics is out of network. The name on your insurance card may differ from the insurer’s name, and network status can vary by plan and location.
Call the member services number on your insurance card and ask:
- Is Baylor Genetics in network for my exact plan?
- Does my plan cover the genetic test my healthcare provider ordered?
- Does my plan require prior authorization?
Will my test be covered?
Your insurance plan decides whether a test is covered. Coverage may depend on medical necessity, the plan’s medical policy, prior authorization requirements, and your benefits. Prior authorization or an estimate does not guarantee that the plan will cover the test or pay the full cost.
How is my estimated out-of-pocket cost determined?
For patients using insurance, Baylor Genetics can review benefits to help estimate possible out-of-pocket cost when the provider requests an estimate with the test order. Baylor Genetics submits the insurance claim; patients do not file the claim themselves.
Out-of-pocket costs can vary depending on the following factors:
- Your insurance plan
- The test ordered
- Estimated reimbursement
- Your insurance plan’s contracted rates
What if insurance does not cover my test?
If your plan does not cover the test, self-pay and financial assistance may be available. Patients with insurance must use their insurance before applying for financial assistance.
Other Payment Options
Medicaid
Medicaid coverage and prior authorization requirements can vary by state and test. Please contact your Medicaid plan to inquire whether the test is covered and whether prior authorization is required. Baylor Genetics can help with billing questions after the order is submitted.
Medicare
Baylor Genetics is a certified Medicare provider. Medicare has established coverage criteria for genetic tests. If you do not meet Medicare’s coverage criteria, you may still receive testing, but a completed Advance Beneficiary Notice of Noncoverage (ABN) is required. The ABN explains that Medicare may not pay and that you may be responsible for the cost if you choose to continue.

No Surprises Act & Good Faith Estimate
Surprise billing is an unexpected medical bill for care from an out-of-network provider or facility when you did not know the provider or facility was out of network before you received the service. Federal protections may apply in some situations.
If you believe you were wrongly billed, call the No Surprises Help Desk at 1-800-985-3059 or visit the CMS Medical Bill Rights website. Your state may also offer additional protections. On your state government’s official website, search for “no surprises,” “balance billing,” or “consumer protection.”
Good Faith Estimate
A federal Good Faith Estimate is different from an insurance-based cost estimate. An insurance-based estimate uses your health plan information. A Good Faith Estimate is provided under federal rules for eligible patients who do not have insurance or are not using insurance.
Under the law, healthcare providers must give patients who do not have certain types of health coverage—or who are not using certain types of health coverage—an estimate of the expected cost of healthcare items or services before those items or services are provided.
- You have the right to receive a written Good Faith Estimate for the total expected cost of healthcare items or services when you request an estimate or schedule the items or services. This includes related costs such as medical tests, prescription drugs, equipment, and hospital fees.
- If you schedule a healthcare item or service at least three business days in advance, your healthcare provider or facility must give you a written Good Faith Estimate within one business day after scheduling.
- If you schedule a healthcare item or service at least 10 business days in advance, your healthcare provider or facility must give you a written Good Faith Estimate within three business days after scheduling.
- You may also ask a healthcare provider or facility for a Good Faith Estimate before scheduling an item or service. The provider or facility must give you a written Good Faith Estimate within three business days after your request.
- If a provider or facility bills you at least $400 more than the amount included in the Good Faith Estimate from that provider or facility, you may be able to dispute the bill.
- Save a copy or picture of your Good Faith Estimate so you can compare it with your bill.
For questions or more information about your right to a Good Faith Estimate, visit: www.cms.gov/nosurprises/consumers, email [email protected], or call 1-800-985-3059.

Billing Terms
These definitions can help you understand common billing and insurance terms you may see.
Deductible:The amount you pay for covered healthcare services before your insurance plan begins to pay, based on your plan rules. |
Coinsurance:The percentage of a covered healthcare cost you pay after you meet your deductible. |
Copayment:A fixed amount you pay for a covered service from a provider. |
Allowable charge (allowed amount):The maximum amount an insurance plan uses to calculate payment for a covered service. It may also be called an eligible expense, payment allowance, or negotiated rate. |
Prior authorization:A decision your health insurer may require before a service to confirm that it is medically necessary. Prior authorization is not a promise that the plan will cover the cost. |
Explanation of Benefits (EOB):A statement from your insurance company that shows how it processed a claim. An EOB is not a bill. |
Current Procedural Terminology (CPT) code:A five-character alphanumeric identifier published by the American Medical Association (AMA) that represents a specific medical, surgical, or diagnostic service or procedure. CPT codes are used to describe healthcare services so they can be consistently reported and billed across the U.S. healthcare system. |
ICD-10 code:A code used to describe a diagnosis, condition, symptom, or reason for a healthcare service. Insurance plans may use ICD-10 and CPT codes when reviewing claims. |
Advance Beneficiary Notice of Noncoverage (ABN):A notice that explains that Medicare may not pay for a service and that you may have to pay if you choose to continue. |
