Please include the test code of interest, the ordering healthcare facility, and the patient’s insurance plan when contacting our team.
CPT codes are available for each test on our test menu in the billing information section of the page. CPT codes provided are based on AMA guidelines and are for informational purposes only. Please direct any questions regarding CPT codes to our Billing team at firstname.lastname@example.org
Yes, you can submit a request using our online portal or by emailing email@example.com or firstname.lastname@example.org. Please provide patient information, test information, and insurance information when contacting our team
Yes, we do. Any patient experiencing financial hardship may qualify for our Patient Assistance Plan. To learn more about this program click here.
All existing balances or pre-payments can be processed using our payment portal. We also accept credit cards, checks, money orders, and wire transfers. If you would like to pay over the phone, please call our Billing team at 800.411.4363, ext. 2.
We are contracted with most insurance plans, including Medicare and Medicaid. For a full list of our contracted plans, click here.
A VOB can take up to two business days. This process can be extended if prior authorization is required/requested.
We can provide self-pay pricing via phone at 1-800-411-4363. Institutional and insurance pricing will require more information and investigation from our team.
Baylor Genetics strives to address billing matters quickly. However, more complex issues may require additional research. For these matters, please allow up to 5 days for processing.

Providers and institutions with access to our billing portal can make payments online. However, automated payments are not currently an option.
