The price of a test varies and depends on the type of test and methodology used. Please contact our Billing Team at firstname.lastname@example.org to obtain pricing information. Please include the test code of interest, the ordering healthcare facility, and your insurance plan name when contacting our team.
CPT codes are available for each test on our test menu in the billing information section of the page. CPT codes provided are based on AMA guidelines and are for informational purposes only. Please direct any questions regarding CPT codes to our Billing team at email@example.com.
Yes. Your provider can submit a request on your behalf by using our online portal or by emailing firstname.lastname@example.org or email@example.com. When emailing, please provide the patient information, test information, and insurance information.
