Test Cancellation Policy

Baylor Genetics can cancel testing within 24 hours of sample receipt at the request of the institution. If there is missing information that prevents Baylor Genetics from completing the billing process, you will be notified and given 5 business days to resolve the issue. Otherwise, testing will be cancelled. All efforts will be made to communicate any missing information.

Cancellation requests must be received by 5 p.m. the day after sample receipt for the request to be approved and processed. To cancel a test, please submit one of the following:

1. A signed Test Revision Authorization Form (TRAF)
   • Form must be fully completed
   • Signed by the ordering institution
   • Faxed to 1.800.434.9850
2. A letter with the test cancellation request on an official letterhead

A cancellation report will be faxed to all referrals listed on the original requisition. For assistance or questions regarding test cancellation, please call 1.800.411.4363.

Cancellation Due to Financial Matters

Baylor Genetics Test Cancellation Policy includes tests cancelled due to financial matters. If the financial criteria are met, the test will be performed. Tests that do not meet the criteria listed below will be cancelled.

Reasons for Financial Cancellation:

• Self-Pay samples without payment
• International samples without payment
• Insurance samples without complete billing information (including diagnosis codes and ordering provider’s name)
• Non-contracted out-of-state Medicaid or welfare programs
• Medicare samples without required ABN forms (English | Spanish)
• HMO or Medicaid HMO samples without required authorization
• Non-contracted insurance carriers
• Financially abandoned samples
• Non-established institutions requesting institutional billing terms
